Extension-Based Lower Back Pain Progression is an evidence-based progression for patients who have sustained an extension based lumbar injury. This progression has been formulated by physicians, physical therapists, athletic trainers, and strength coaches with extensive adolescent and adult sports medicine experience.
Extension-Based Lower Back Pain Progression should only be followed after a diagnosis and order for Physical Therapy is made by a qualified MD. Progress on this program under the supervision of a PT or ATC/LAT. The progression has 3 phases which the patient progresses on based on healing. Progressing from one phase to another is determined by the referring physician and medical team.
Progression through each phase is based on the diagnosis made by your M.D..
Diagnoses can vary strain(muscle), sprain( ligament), stress fracture (Bone)
**Physician can speed or delay progression based on diagnostic and clinical findings.

-Phase 1 Exercises are suggested to be done 5 days per week with stoppage of your injurious activity or sport.
-Progress through each exercise trying to build up to 2 sets of 25 repetitions.
-Progress through each stretch holding 30 seconds, 2 times.
-Aerobic activity allowed during stage 1: biking or walking can build to 30 minutes 3-5x's per week.
Phase 2 Exercises - can begin at 8 weeks post-diagnosis and stoppage of your injurious activity or sport (if your MD, PT or AT feel appropriate progress has been made).
- Phase 2 exercises are suggested to be done 3-5 days per week.
- Aerobic activity allowed during stage 2: biking, walking, or elliptical, can build to 30 minutes 3-5x's per week.
Phase 3 Exercises -can begin at 12 weeks post-diagnosis and stoppage of your injurious activity or sport (if your MD, PT or AT feel appropriate progress has been made).
- Phase 3 exercises are suggested to be done 3x's days per week, non consecutive days
- Aerobic activity is allowed during stage 3: biking, walking, elliptical, or running* can build to 30 minutes 3-5x's per week. *Need MD clearance to run.
